CUSTOMISED BARIATRIC SOLUTIONS

ONE SIZE DOES NOT FIT ALL!

We do not recommend one solution (for e.g., sleeve gastrectomy) to all patients who are obese. As obesity is a complex, multifactorial disease, for best results, its treatment also needs to be tailored to each individual. The different 'OBESITY SOLUTIONS' described here is to highlight that “all obese individuals are not the same.” For the sake of simplicity, we divide them, to three main groups, to explain and plan their weight loss solutions.

  1. Some have greater fat with lesser metabolic effects - see BARIATRIC PRO.
  2. Some have lesser fat with greater metabolic effects - see BARIATRIC MET (MET- short for METABOLIC).
  3. Some are mildly obese (less than 35 BMI), but are anxious that the weight gain may go out of control, and seriously desire to be fit. We categorize them under BARIATRIC LITE, category.

BARIATRIC PRO

NON-DIABETIC OBESITY

an obese white man in red tshirt

"Are you worried about your increasing weight gain and had multiple failed weight loss attempts (diet & exercise) in the past? Are you now serious for a reliable and a longterm solution?"

If your BMI is over 35, and have no diabetes, with any of the following:

  • You have several failed weight-loss attempts.
  • You have issues of knee and back strain.
  • You have sleep disorder / daytime sleepiness or snoring.
  • You have other issues related to obesity.
  • You have no previous history of bariatric surgery in the past.

You are a candidate for surgical BARIATRIC PRO weight loss:

THE RECOMMENDED OPTIONS ARE:

  1. SLEEVE GASTRECTOMY
  2. GASTRIC BYPASS
  3. OTHER OPTIONS

The final decision of your procedure is what you choose  after you have considered Dr. Zachariah's recommendations.

BARIATRIC MET

OBESITY WITH DIABETES

"In addition to obesity, are you also having problems (that cause reduced lifespan) like diabetes, BP, high cholesterol, etc? Are your medications not helping you?"

If your BMI is above 30, and have uncontrolled Type 2 diabetes and with any of the following:

  • You have raised cholesterol or any abnormal lipid levels.
  • You are on insulin for Type 2 diabetes.
  • You have a fatty liver with inflammation.
  • You have polycystic ovarian syndrome.
  • You have other issues related to obesity.
  • You have no previous history of bariatric surgery in the past

You are a candidate for surgical BARIATRIC MET weight loss:

It is likely that your obesity, even though milder, has seriously affected on your metabolism. One such effect is “insulin resistance” causing diabetes. Hence, you will benefit with procedures that have a greater resolution of diabetes. These procedures alter the levels of gut hormones and produce insulin-like effect without stressing the already ‘weak’ pancreas, hence are better than the commonly done sleeve gastrectomy. They typically have a “bypass” element on the intestine, hence, need special follow up for their nutritional side-effects.

THE RECOMMENDED OPTIONS ARE:

  1. MINI GASTRIC BYPASS
  2. CLASSIC GASTRIC BYPASS
  3. ‘SLEEVE PLUS’ OPERATIONS
  4. SLEEVE GASTRECTOMY

The final decision of your procedure is what you choose, after you have considered Dr. Zachariah's recommendations.

BARIATRIC LITE

MILD OBESITY

An obese women in red dress

"Are you hesitant to undergo an operation, but want to start a lifestyle change, with a weight loss procedure? Maybe, just to be fit?"

If your BMI is between 30-35, and have no health issues, but

  • You are concerned about 'losing control' on the steadily gaining weight.
  • Diet and exercise are not keeping the weight down.
  • You have family history of metabolic diseases like diabetes, BP, raised cholesterol, etc.
  • You are looking to have only a temporary procedure, to initiate your weigh loss efforts.

You can choose to have the BARIATRIC LITE option.

Majority of people (50-60%) today, are overweight, or in the Class 1 obesity ( BMI 30-35) section. Low calorie diet with exercise, is the mainstay of the weight loss in this group. However, for most, it is easier said than done. So, we recommend this solution only for a temporary weight loss, as there is a potential for weight-regain, if you do not continue an effective weight loss (diet and exercise) program after that. Hence, you should consider the pros and cons, before choosing this option. However, many do report that they could jump-start their weight loss programs, this way.

THE RECOMMENDED OPTIONS ( ALONG WITH DIET + EXERCISE) ARE:

  1. GASTRIC BALLOON
  2. GASTRIC BALLOON + INJ. SAXENDA
  3. OTHER OPTIONS

The final decision of your procedure is what you choose  after you have considered Dr. Zachariah's recommendations.
